PAPERWORK :
All paperwork is due by Sunday, May 27th and accessible below.
Please complete BOTH copies and have each notarized and return to Brian by May 27th.
Your student will have the option of choosing what type of ministry they would like to work
in for the week. They can choose from Children’s, Social, Creative, GAR (Games & Rec),
PCY (Painting - Construction - Yardwork) and Homeless Ministries. I need to know their top
3 choices by Sunday, May 27th with #1 being what they most want to do and #3 being what
they least want to do. Descriptions of these Ministry Tracks are provided via the above link.
Please complete only if your student has a special need (emotional or physical) that needs
to be addressed or that we need to be aware of at camp. This card will only be seen by
Brian and your students Bible Study Leader while at FUGE.

Please provide 2 copies (front & back) of your students insurance card and give to Brian
by May 27th.

PARENT DEVOTIONALS :
This devotional is a way for you to experience what your student is learning at camp and gives you the opportunity to discuss what you both have learned once your student gets back from New Orleans.
FINAL BALANCES :
All balances will need to be paid by Wednesday, May 30th. I will contact you directly to let you know your students final balance on May 28th.
8x10 Funds : I cannot apply 8x10 funds to student balances until all the money has been collected. The last day to get the 8x10 contributions to the church is Sunday, May 27th. I will apply what we raised thru the 8x10 as well as what we received from the Battle of the Sexes Contest on Monday, May 28th and will contact you directly to let you know your students final balance.
